FY27 Block Grant Request for Responses (RFR)

We are excited to share the Request for Responses (RFR) for Southwest Washington Mental Health and Substance Use Prevention, Treatment and Recovery Services (formerly SABG) Block Grants. With this RFR, Carelon Behavioral Health seeks to support innovative programs that promote integrated care delivery for priority populations.

All agencies, even those currently funded with Block Grant funds, will need to submit an application in order to be considered for block grant funds beginning July 1, 2026.

This RFR includes both Mental Health Block Grant (MHBG) and Substance Use Prevention, Treatment and Recovery Services Block Grant (SUPTRS) funding.

  • Mental Health Block Grant (MHBG) – The program’s objective is to support the provision of comprehensive community mental health services for adults diagnosed with Serious Mental Illness (SMI) and children diagnosed with Serious Emotional Disturbance (SED).
  • Substance Use Prevention, Treatment and Recovery Services Block Grant (SUPTRS) – The program’s objective is to plan, implement, and evaluate activities for the prevention, treatment, and recovery of substance abuse to support individuals in need of these services.

YOUTH NAVIGATOR PROGRAM

The Southwest Youth Navigator program serve the counties of Clark, Skamania and Klickitat, ensuring that youth and families in these areas have access to essential behavioral health resources and support. Our programs aim to place children, youth, and families at the center of its efforts, collaborating with stakeholders to ensure comprehensive care and support. Since March 2023, the two programs have successfully served over 330 youth and families.

SOUTHWEST DIVERSION HOUSING PROGRAM

The Southwest Diversion Housing Program supports individuals in Clark County facing behavioral health challenges and housing instability. Launched in 2024, this initiative is designed to assist those transitioning from crisis stabilization or diverted from criminal prosecution. Our team of dedicated housing specialists offers temporary housing, financial assistance, and barrier removal strategies to help clients secure stable, long-term housing. With over 230 referrals and 200 individuals served, we are committed to enhancing quality of life through strategic partnerships with 10 referring agencies and a wealth of essential resources.

COMITÉ DEL PROGRAMA PARA PACIENTES INTERNADOS A LARGO PLAZO PARA NIÑOS (CLIP)

El Comité del Programa de Hospitalización a Largo Plazo para Niños del Sudoeste de Washington (SWWA CLIP) está compuesto por varios representantes de agencias y miembros de la comunidad que se reúnen mensualmente para ayudar a guiar a los jóvenes y las familias a navegar por el tratamiento psiquiátrico para pacientes hospitalizados más intensivo y financiado con fondos públicos disponible para ellos. Programas de justicia penal

Carelon Behavioral Health contracts with local providers to deliver services to individuals with a behavioral health need who are involved with the local criminal justice system or incarcerated in the local jails. Services are provided within the rules of the two state proviso funding sources: Criminal Justice Treatment Account and Jail Transition.
