FY27 Block Grant Request for Responses (RFR)
We are excited to share the Request for Responses (RFR) for Southwest Washington Mental Health and Substance Use Prevention, Treatment and Recovery Services (formerly SABG) Block Grants. With this RFR, Carelon Behavioral Health seeks to support innovative programs that promote integrated care delivery for priority populations.
All agencies, even those currently funded with Block Grant funds, will need to submit an application in order to be considered for block grant funds beginning July 1, 2026.
This RFR includes both Mental Health Block Grant (MHBG) and Substance Use Prevention, Treatment and Recovery Services Block Grant (SUPTRS) funding.
- Mental Health Block Grant (MHBG) – The program’s objective is to support the provision of comprehensive community mental health services for adults diagnosed with Serious Mental Illness (SMI) and children diagnosed with Serious Emotional Disturbance (SED).
- Substance Use Prevention, Treatment and Recovery Services Block Grant (SUPTRS) – The program’s objective is to plan, implement, and evaluate activities for the prevention, treatment, and recovery of substance abuse to support individuals in need of these services.

YOUTH NAVIGATOR PROGRAM
The Southwest Youth Navigator program serve the counties of Clark, Skamania and Klickitat, ensuring that youth and families in these areas have access to essential behavioral health resources and support. Our programs aim to place children, youth, and families at the center of its efforts, collaborating with stakeholders to ensure comprehensive care and support. Since March 2023, the two programs have successfully served over 330 youth and families.
SOUTHWEST DIVERSION HOUSING PROGRAM
The Southwest Diversion Housing Program supports Clark County individuals experiencing behavioral health challenges and housing instability who are transitioning from crisis stabilization or participating in diversion programs. Since launching in 2024, we have received 380+ referrals and served 270+ individuals, partnering with 15 agencies and utilizing more than 50 housing locations across the community. Through housing navigation, financial assistance, barrier removal, and strong community partnerships, we help individuals achieve greater housing stability and improved quality of life.

کمیته برنامه بلندمدت بستری کودکان (CLIP)
کمیته برنامه طولانی مدت بیماران بستری کودکان جنوب غربی واشنگتن (SWWA CLIP) متشکل از نمایندگان آژانس های مختلف و اعضای جامعه است که به صورت ماهیانه گرد هم می آیند تا جوانان و خانواده ها را در جهت هدایت درمان های روانپزشکی بستری که با بودجه عمومی در دسترس آنها است ، راهنمایی کنند. خدمات CLIP برای جوانان بین 5 تا 18 سال طراحی شده است. درخواست ثبت نام CLIP منطقه به منطقه توسط کمیته محلی CLIP انجام می شود.

Carelon Behavioral Health contracts with local providers to deliver services to individuals with a behavioral health need who are involved with the local criminal justice system or incarcerated in the local jails. Services are provided within the rules of the two state proviso funding sources: Criminal Justice Treatment Account and Jail Transition.
